Diploma and Transcript Requests
Requests for Diplomas
Requests must include:
- Name at the time of graduation
- Year of graduation
- Current mailing address
- Phone number
Requests can be e-mailed to lambertk@mhs-pa.org
NOTE: You do not receive a copy of your original diploma — you will be receiving a new diploma with the signatures of current school administrators.
Requests for Transcripts
Requests for Transcripts should be directed to Mrs. Debra Belack in the office of Enrollment Management & Family Relations.
Request must include:
- Name and year of graduation
- Social Security number
- Date of birth, signature
- Whether an (official or unofficial) transcript is needed
- Where it must be sent (address of employer, college, etc.)
